We at Memorial Hospital believe every patient should have easy, instant access to his or her important health information at any time.
That’s why we’re pleased to be able to offer the FollowMyHealth Patient Portal. This allows you to access your lab reports, radiology (x-ray) reports, discharge instructions and more on a computer, laptop, tablet or even your smart phone. You can even print, fax or e-mail your reports to someone else.
All you need to be able to participate in the portal is a valid e-mail address. We recommend not using a work or school e-mail since they often have restrictions against receiving non-work-related e-mails and once you access the portal, your information is not guaranteed to be held private.
At the time of your next visit to Memorial Hospital, make sure a current, valid e-mail address is on file. As soon as you are discharged, an invitation to join FollowMyHealth will be sent to your e-mail. Follow the instructions to get set up and you’ll be able to access information going back to October 1, 2017.
